On-Campus Flu Shots at AIMS Education in Piscataway
Saint Peter’s University Hospital Community Health Services will be setting up a flu clinic at the AIMS Education Piscataway campus on Friday, November 13th from 12:00pm to 1:30pm. Flu shots will be available to students and staff. Saint Peter's will bill your insurance company. For those without health insurance, the standard flu shot will cost $25 and the high dose flu vaccine will cost $50.
There will be a sign-up sheet at the front desk in Piscataway. Students and staff can sign up in person or by calling the Piscataway front desk at Ext. 300. An email containing a packet of required flu forms was emailed to you. The forms must be filled out prior to your appointment. If you did not receive the email, the forms are available here.
Please only bring your insurance card and completed packet of flu forms to the flu clinic. Leave your coat, sweater, pocketbook, or anything else you have in your car (excluding a cane or walker).
Saint Peter's has asked that you complete all forms and follow the checklist below prior to your arrival at the flu clinic to ensure that all CDC guidelines are followed.
- Complete the Influenza Consent form 2020
- Check the vaccine assessment questions located on the back of the influenza consent form.
- Complete the Insurance Information Sheet
- Complete the top half of the Dear Doctor Form
- Bring a photocopy of your primary insurance card (both front and back).
- If you are not using insurance, please have a check written out to SPUH-CHS, or exact cash. Saint Peter's does not accept electronic payment.
Saint Peter's will be adhering to the Centers for Disease Control and Prevention guidelines on infection prevention. Social distancing will be enforced during the flu clinic and face masks are required. A temperature check and health screening will be conducted when you arrive, and you will be asked to sanitize your hands.
