Physical Exam, TB Screening, and Immunizations Checklist
Health Forms
Required health forms are provided in each program's student health packet. The packets include a Physical Examination form, Tuberculosis (TB) Screening form, and an Immunizations Checklist. Each program may have different requirements, so it is important that you select the appropriate packet for your program.
Exempt Program
The student health packet is not required for students enrolled in the Medical Billing and Coding program.
Magnus Health
AIMS Education has partnered with Magnus Health to assist with the collect of health documents. All new enrollments will receive a welcome email from Magnus Health. All student health documents and immunization records must be uploaded directly to Magnus Health. If you've already received the welcome email and created your password, you can access you account at the link below.
Student Health Packet A
Students enrolled in the Anesthesia Technician, Cardiac Monitor Technician, Cardiovascular Invasive Specialist, Cardiovascular Technology, Diagnostic Cardiac Sonography, Diagnostic Medical Sonography, Endoscopy Technician, MRI Technology, Neurodiagnostic Technology, Pharmacy Technician, Sterile Processing Technician, and Surgical Technology programs are required to complete Student Health Packet A. Student Health Packet B
Students enrolled in the EKG/Phlebotomy, Medical Assistant, Patient Care Technician, and Phlebotomy programs are required to complete Student Health Packet B. Meningococcal Forms
New Jersey state law requires new students enrolling in an institution of higher education in New Jersey to have received the meningococcal vaccines as recommended by the Advisory Committee on Immunization Practices (ACIP).
The questionnaire below is used to determine if you are required to required to get either of the meningococcal vaccines. Students who are required to receive the MenACWY and/or MenB vaccination must submit proof of vaccination in their Magnus Health account.
